Applications and Forms

Applications for the Academy must be filled out completely, signed and the following documents must be presented along with the application and all fees paid, if applicable.

Birth Certificate
Immunization Records
Proof of Residence (Must be a NC Resident at time of applying)

The following documents must be present in your child’s file.

 Student Information Forms  these forms must be filled out and turned in the first day of school or class. This document contains medical and personal information in the event of an emergency. All forms must be signed by the parent/guardian.
