The following documents must be present in your child’s file.
Sterling’s Student Information Form for the current academic year is electronic. You will be asked to create a new account and log-in. You can use the same account for multiple children, but each family will have one account. This form must be filled out and submitted prior to the first day of school or class. This document contains medical and personal information in the event of an emergency. Contact Josie Spreher for additional information.
Parental Permission to Administer Medication – This form has to be filled out for “All” medications, prescription and over-the-counter. One form for each medication. If your child has asthma or a Epi-Pen, with a food allergy, there are additional forms below that must be attached. Student will not be allowed at school until all required medication is on hand at school. (Remember, if your child has an Epi-Pen, or Inhaler and is in the Before/After school program, we require additional medication for those classrooms as well.) These forms do NOT require a physician’s signature.
