The following documents must be present in your child’s file.
Sterling’s Student Information Form for the current academic year is electronic. You will be asked to create a new account and log-in. You can use the same account for multiple children, but each family will have one account. This form must be filled out and submitted prior to the first day of school or class. This document contains medical and personal information in the event of an emergency. Contact Josie Spreher for additional information.
Permission to Administer Medication – This form has to be filled out for all medications, both prescription and over-the-counter. A separate form should be filled out for each medication. If your child has asthma, requires either an Inhaler an Epi-Pen, or has a food allergy, there are additional forms below that must be submitted. Students will not be allowed at school until all required medication is on hand to administer these to your student. (Please note that if your child has an Epi-Pen, or an Inhaler and is in the Before/After school program, we require additional medication for those classrooms as well.) These forms do NOT require a physician’s signature.
Additional Contracts and Forms
In order to accompany your child on overnight field trips, Sterling requires that you order a background check through CastleBranch. Log onto the external link above; Sterling’s package code is: ST74
Please be aware that this should be done a few days prior to your child’s trip, in order for your request to be approved.
